For Children:

The Weld County Public Health Department provides routine immunizations for children, including immunizations that are required by Colorado School Law. Immunizations are available for children 0-18 years of age through the Vaccine for Children program.

Community education and immunization resources are also available. Please call (970) 304-6420, ext. 2308 if you would like more information.

If unable to make your appointment please call to cancel, so it is available for someone else.

A parent or legal guardian must be present for all children under the age of 18 years old or a signed Weld County Health Department consent form must be filled out prior to the appointment.

Payment requested at time of service. Cash, check and credit cards accepted. Private insurance is accepted, additional fees do apply, see immunization fees. No child is turned away for routine vaccines due to inability to pay.

If Medicaid eligible, please bring Medicaid card to visit.

Please bring all past immunization records. Child will not be seen without past immunization records (unless prior authorization is granted.)

 

AUTHORIZATION FOR RELEASE OF INFORMATION

All requests for the Authorization For The Release of Information for Medical Records must have picture ID to verify their identity.
