For Children:

The Weld County Public Health Department provides routine immunizations for children, including immunizations that are required by Colorado School Law. Immunizations are available for children 0-18 years of age through the Vaccine for Children program.

Immunization Clinic Hours: (by appointment only)

• Please call 304-6420 to make appointment or for questions.
• If unable to make your appointment please call to cancel, so it is available for someone else.
• Friday walk-in hours are first come first serve.
• A parent or legal guardian must be present for all children under the age of 18 years old or a signed Weld County Health Department consent form must be filled out prior to the appointment.
• Payment requested at time of service. All routine childhood immunizations cost $14.70 per vaccine. Cash or check accepted, no credit cards please. The fee for children's vaccines may be waived for those unable to pay. No child is turned away for routine vaccines due to inability to pay.
• If Medicaid eligible, please bring Medicaid card to visit.
• Please bring all past immunization records. Child will not be seen without past immunization records (unless prior authorization is granted.)
