Partnerships - Weld County Human ServicesOne of the goals of the Weld County Department of Human Services is to continuously strengthen services and support innovation that leads to a culture that empowers people to improve their quality of life. Often this is done through seeking out and forming partnerships and collaborations with other agencies, organizations, and members of the community. Below you will find a brief overview of some of these forward-looking partnerships.

Weld County Collaborative Management Program – HB 1451

On July 1, 2005, Weld County Commissioners and key partner agencies came together to establish the Weld County HB04-1451 Collaborative Management Program (1451 CMP).  The goal is to better serve children, youth and families who are involved in, or are at risk of becoming involved in, the child welfare or juvenile justice systems and who are receiving, or would benefit from, integrated multi-agency services.

Goals: 
  • Reducing truancy;
  • Reducing juvenile recidivism;
  • Meeting the needs and services of children, parents and foster parents;
  • Improving the well-being and permanency outcomes for children and youth whose permanency has been negatively affected by meth and other drug use.

Partnering Agencies:

The Weld County 1451 CMP operates under the Youth and Family Connections agency (formerly known as the JAC) and includes leaders and representatives from the Weld County Department of Human Services, Weld County Health Department, Weld County Commissioners, Weld County District Attorney 19th Judicial District, Greeley-Evans School District #6, Weld BOCES, North Range Behavioral Health, Island Grove Regional Treatment Center, 19th Judicial Probation Department, Division of Youth Services, Weld School District RE-4 and United Way of Weld County.

Weld Faith Partnership

The Weld Faith Partnership seeks to address community issues through the development of a proactive, collaborative approach that will assist at-risk families in accessing support through community faith-based organizations, government entities, non-profit human service agencies and local businesses.

Weld Faith Partnership Council

The Weld Faith Partnership Council acts as a liaison between the faith-based community and the Weld County Board of Commissioners, to make informed strategic recommendations from a faith-based perspective to the Board of Commissioners regarding issues of concern to the community. Applications are open to all residents of Weld County and appointments are made by the Board of Commissioners.
