DEATH CERTIFICATES - VITAL STATISTICS

The Death Certificate is filed with the Health Department by the Funeral Director, after the coroner has determined the cause and manner of death. Should you require certified copies, they can be ordered through the funeral director or they may be purchased from:

PROOF OF DEATH LETTER

Proof of Death Letter is a certified letter documenting a decedent’s death. Information includes name of decedent, date and place of death. This letter does not substitute a Death Certificate.May be requested through the Document Inquiries.

AUTOPSY REPORT REQUEST

Coroner’s Autopsy Report is public information and may be requested, a fee may apply. Family members are granted one copy for FREE and all other request and non-family members are subject to a $.25 per page fee. Payment is due prior to obtaining report. May be requested through the Document Inquiries.

BURIAL ASSISTANCE

Families may find themselves financially unable to cover costs related the final disposition of their loved one. Weld County offers cremation for individuals with little or no income or resources. To quailify, both the decedent and the person legally responsible for the disposition of the remains must be indigent. Please contact Weld County Social Services Phone: (970)352-1551 to apply for burial assistance.
