The Colorado Board of Health requires that certain diseases be reported by laboratories and physicians. Information about what diseases must be reported in Colorado can be found at the Colorado Department of Public Health and Environment webpage.
To Report a Disease:
If you are a physician or a laboratory technician and would like to report a disease to the Weld County Health Department, please fill out the Disease Report Form and fax to (970) 304-6412 along with the lab results that prompted you to report this disease. Please tell your patient that the Health Department will be contacting them for a follow-up.
