Baby Care/Kids Care and Pregnant Woman’s Medicaid 

Eligibility is based on gross income and family size.  Family income must be less than the need standard.  The unborn child is considered a part of the household.  Income can be verified for the current or preceding month depending upon the date of application. 

There is no resource (asset) limit for Family Medicaid and Pregnant Women's Medicaid. 

Medical benefits may be backdated up to 90 days from the date of application.  Income must be verified for the calendar month, which includes the date of service within the backdated 90-day period.

 You may apply for Medicaid at any of the following sites:

Applications received at the above sites are sent to Weld County Department of Human Services for determination of eligibility.

The intake hours for the Weld County Department of Human Services are from 8:00 am to 2:30 pm Monday through Friday on a walk in basis.

If the family income and/or resources are above the need standard, the application for children will be referred to Child Health Plan Plus (CHP+). 

The Colorado Indigent Care Program (CICP) is available for adults only at either Sunrise Community Health Center or one of the Salud Clinics.  For more information, contact either clinic.

Participants must choose a Primary Care Physician (PCP) or have an HMO (Colorado Access).  Colorado Access will refer recipients to participating clinics:  Monfort Children’s Clinic, Sunrise Community Health Center, North Colorado Family Medicine (NCFM), and Salud Clinics in Fort Lupton and Frederick.

A co-pay is required for adults only by the PCP.  There is no co-pay for Colorado Access recipients.
